Please release the number of a) Serious Untoward Incidents and b) Adverse Incidents attributed to i) foreign-trained locums and ii) foreign-based locums who were over in the UK for shift work in each of the last three years (financial or calendar, whichever is easier).
For each SUI and each Adverse Incident, please release a) what went wrong, b) what happened to the patient, c) the nationality of the locum, d) how long they had been at the trust before the incident occurred, e) whether they are still working at the trust.
For each SUI and each Adverse Incident, please release the resulting report submitted to the SHA in full, subject to data protection redactions.

I would like to make a Freedom of Information request concerning alcohol consumption and elective surgery.

1. Does the hospital do anything to actively encourage patients to reduce their alcohol consumption before elective surgery, particularly if they are heavy drinkers.

2. Does the hospital ever screen patients for alcohol prior to elective surgery

3. Are their any circumstances under which a patient's operation would be postponed if he/she was found to be drinking at dangerous levels.

1. The total amount of money spent annually on away days for staff in 2006, 2007, 2008 and 2009 respectively. (Away days include any trips taken by staff, such as team building exercises, conferences, meetings, charity days, study trips, corporate trips, executive events, research trips, fact-finding trips, etc.)

1.b. For each individual trip, please itemise the cost, the purpose of the trip, the number of staff taking part, the number of executive officers taking part, the address and location travelled to, and the dates travelled.

Please provide details of the highest ten sums that your trust has paid out in the past 12 months in compensation to patients who have made a complaint against the trust.
In each case please make clear the total amount paid to the patient, the reason for the payout, the total amount of legal fees paid and the solicitor/legal firm to whom the fee was paid.
