Anticoagulant management service

The haematology department based at the central pathology laboratory is responsible for the provision of the UHNS anticoagulant management service, (AMS), which initiates and monitors patients on oral anticoagulants.  The most commonly used drug is Warfarin.

Patients requiring oral anticoagulant therapy can now be referred to the AMS. This includes patients who have to restart Warfarin after temporary discontinuation and patients already taking Warfarin on admission.

As a patient this means that if you are admitted to hospital, your Warfarin treatment will continue to be managed by the AMS if authorised by your clinician. The seven day service provides the following:
 

  • Same day review of patient by nurse specialist (provided faxed referral is received by 3.30pm weekdays and 10.00am Saturdays and Sundays).

  • Counselling, providing written patient information.

  • Checking INR and advising on Warfarin dose during the entire in patient stay.

  • Arranging follow up appointment on discharge.
