Elderly care and frail elderly

We aim to provide a rapid and full multi-disciplinary assessment for all patients who are screened as requiring frail elderly services.

Services for frail elderly patients  are provided on the frail elderly assessment unit on Ward 210 , Ward 79 and Ward 123 (female admission wards) and ward 121 (male admission wards). Geriatricians provide services for older people in the Trust and also work within intermediate care and rehabilitation facilitates in wards managed by Staffordshire and Stoke on Trent Partnership Trust.    

Each ward has a team of staff which include Consultant Geriatricians, Specialist Registrars, Speciality Doctors, Foundation Doctors, GP Trainees, Core Medical Trainees, Advanced Nurse Practitioners, Senior Clinical Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dieticians and Pharmacists, Lead Nurses, Specialist Assessment Nurses and Social Workers. Some of these staff are dedicated to each ward and thus provide a continuous seamless service for the patient.
