Palliative care improves quality of life for patients and families who face terminal illnesses. It encompasses pain and symptom relief and spiritual and psychosocial support. Palliative care is important from initial diagnosis right up to end of life and is carried out by all health care professionals in some form.
The hospital palliative care team provide advice and support to patients and staff at University Hospital excluding paediatrics. The service is available for all patients with palliative needs, irrespective of diagnosis.
Our palliative care team liaise with teams in other organisations to ensure your needs are catered for properly and the best possible standards reached. This liaison may involve things like your referral to the most appropriate hospice, home or community hospital for you.
University Hospital's palliative care team has successfully implemented two programmes, namely the Liverpool Care Pathway and the Support Care Plan. These are advanced care plans, or communication tools, to ensure you are cared for in the most sensitive, efficient and beneficial way.
